ENROLLMENT CHECKLIST
• View your current benefit selections and make any desired changes
(If no changes are made online, you will keep your current benefits plan.)
• Enroll in Flexible Spending Accounts
(If no changes are made online, you will not be enrolled in the FSAs.)
• Complete the Health Risk Assessment
(You must do this every year to receive the enhanced wellness and preventive services benefit for you and your covered dependents.)
• Designate your tobacco/nicotine status
(If you do not complete the designation, it will default to tobacco/nicotine use which will result in higher premiums for the voluntary life insurance coverages.)
• Visit the UMR and Ameritas websites to learn about our medical and dental carriers
