A message from Noel Ysasi, Ph.D., CRC:

I am a faculty member at NIU conducting research and I am in need of your participation in this study. Below, you are provided a link to an online survey that will take you approximately 15-20 minutes to complete. In this study, you will be asked to provide your honest thoughts regarding physical disabilities and mental health disorders.

Your participation in this research study will allow you to be placed in a drawing for a $500.00 gift card. Although you will be allowed to enter your contact information for the drawing, this does not take place until the end of the survey and you will only be asked for your email. So no identifying information will be collected and this study is confidential and anonymous. If you are under the age of 18 and/or not a student in higher education, please disregard this request for participation in the study.

You may withdraw at any time, should you feel the need to. The results will help better understand what people honestly believe about people with disabilities.

Participation in this study involves:
• A time commitment of 20 minutes
• Opportunity to win $500.00

For more information about this study, please contact the principal investigator, Dr. Noel Ysasi, by phone at 815-753-1484 or email at nysasi@niu.edu
