The annual NUFlex enrollment period ends this week. University employees have until 5 p.m. Dec. 7 to make changes to benefits options for 2013 and complete the Wellstream Health Risk Assessment.
Employees will not be able to enroll or make changes to benefits after the deadline. If no changes are made, employees will keep current benefits options, not be enrolled in a Flexible Spending Account in 2013, and be assigned a tobacco/nicotine user premium for voluntary life insurance.
NUFlex benefits enrollment should be completed online at https://firefly.nebraska.edu.
Employees opting for the new "employee plus-one" benefits coverage must do so through the UNL Benefits Office. University of Nebraska benefits administrators hoped to offer plus-one enrollment electronically, but in-person registration is required because of complex federal tax regulations.
Employees also have until 5 p.m. Dec. 7 to complete the health risk assessment. To complete the assessment, log on to Firefly and click on the red “Take the Health Risk Assessment” link at the beginning or end of enrollment. The HRA helps individuals evaluate and monitor health. Individuals who complete the HRA receive enhanced wellness and preventive services benefits. To receive credit for HRA completion, the Personal Health Report must be printed or saved.
Also, participation in the Flexible Spending Account does not automatically renew each year so employees must re-enroll during the enrollment period. Employees enrolled in Voluntary Life Insurance must also complete the life insurance tobacco/nicotine designation since premiums are based in part on tobacco/nicotine use.
All benefits changes made during NUFlex enrollment will be effective Jan. 1, 2013.

— Ask questions by contacting the UNL Benefits Office
— Review current benefits by visiting the Firefly website. A confirmation statement showing 2012 benefits is provided in the first step of the NUFlex enrollment process.
— Enroll through the Firefly website by 5 p.m. Dec. 7.
Don't forget to:
— Complete the Wellstream Health Risk Assessment. Employees must do this every year to receive the enhanced wellness and preventive services benefit.
— Enroll in the Flexible Spending Account. Employees must re-enroll every year.
— Designate tobacco/nicotine status. Employees enrolled in the Voluntary Life Insurance must do this every year.
— Provide dependent verification documentation if adding dependents to coverage. Documentation must be received in the UNL Benefits Office by 5 p.m. Dec. 7.
— Complete the online life insurance Statement of Health form if enrolling or increasing any life insurance coverage amounts. Proof of insurability requests must be completed online by 5 p.m. Dec. 7.
— Review the NUFlex confirmation statement to make sure all benefits selections are correct.
