DHHS Background Checks – When is the Notarized Form Requested?

We’ve recently learned of a small number of volunteers who encountered the requirement of the notarized form, even after selecting the online option. There are several reasons why an individual isn’t able to complete the online process and are asked to submit the notarized form. Unfortunately, we won’t know of the exact circumstance that causes it occur in one case vs. another, but common reasons are below. It was also recommended that volunteers not use a cell phone to complete the screening.

The Common Reasons for Notary Verification:
· Minors
· Timeout during online screening
· Common name
· Little/no credit
· Unable to find individual based on given info during first part of screening.

If the notarized form is required, the volunteer will be charged $4 at the time of checkout as opposed to the $5. Those prices are outlined below. Screening fees:
Central Registry Portal Request using Online ID verification= total of $5.00 ($1.00 ID questions; $2.50 DHHS fee; $1.50 payment processing fee)
Central Registry Portal Request using Notary Form= total of $4.00 ($2.50 DHHS fee; $1.50 payment processing fee)
